To register with the practices you must meet the following criteria:
- You must live within our practice boundary (see below)
- You must be resident for at least six months
Practice Boundary:
Diamond Medical Group is a new practice made up of Rutland Lodge Medical Practice, Oakwood Surgery and Shadwell Medical Centre. We are accepting new patients from all of these practice areas and as a result of the merger of these three practices, our practice boundary is as follows:
- The northern boundary runs from Alwoodley Park to Wike and Scarcroft
- The western boundary runs from Scarcroft down to Whinmoor, Hollin Park, Harehills Corner and Sheepscar
- The southern boundary runs from Sheepscar to Burmantofts, around Crown Point and back up to Hyde Park
- The western boundary runs from Hyde Park to Headingley, by Weetwood, Adel East Moor and to Alwoodley Park
Our boundary is shown in the map below. Please click on the map to activate it and to view our practice boundary. Please use the search function on the top right of the map to seach for your postcode to check if your address falls within our catchment area. Enter your postcode and click the magnifying glass and a pin will appear on the map to show your location.
To register with the practices
Diamond Medical Group welcomes new patients. If you wish to register with the practice, you can use one of the following links to register online, depending on which is your preferred branch site:
Alternatively, you can download the forms below and return them to the surgery. If you are not able to download the forms, you may call into the surgery to collect the forms (please adhere to COVID 19 guidelines). The receptionist will provide you with the correct documentation for registering with us.
If you are not ordinarily resident in the UK you may have to pay for NHS treatment (GP services are free of charge) outside of our GP practice. You will be asked to provide proof of entitlement in order to receive free NHS treatment outside of this practice, otherwise you may be charged for your treatment.

Exemptions:
If you have a valid exemption such as an EHIC (not issued in the UK) or have paid the Immigration Health Charge when accompanied by a valid visa then these should be brought with you to the surgery when registering.
Our opening times
What next?
Once we have your completed forms we will process your registration which may take 2 to 3 weeks if you are transferring surgeries, you will still be registered at your previous surgery and can still use them until your registration with ourselves in complete.
Once registered you may make an appointment with any doctor or nurse. We ask all new patients age 15+ to have a health-check which involves taking your medical history, a blood pressure check and a urine check.
All new patients will be registered for online usage to book/cancel appointments, order repeat medications, complete questionnaires and look at records.
